A sanitation employee was cleaning proofer equipment when her right arm was caught by the chain and sprocket resulting in a forearm fracture.

An employee was trying to push out a catch pan to clean an area. The employee's right index finger came into contact with the machine's drive chain and sprocket, causing an amputation to the fingertip. The machine was guarded at the time.

At 12:06 a.m. on April 10, 2021, an employee was preparing tortilla dough for a machine that routinely dropped dough onto a cast pan. While the employee was pulling out the cast pan, the machine's arm struck his hand. The employee was hospitalized, requiring surgery to alleviate pressure.

While packaging tacos on the taco line, an employee's right ring finger was partially amputated.

An employee was moving a loaded lift from an aisle when it became wedged in the aisle and abruptly stopped, throwing the employee. The employee suffered a concussion, and fractures to two neck vertebrae and the left scapula.

An employee was in the basket of an aerial lift while unloading it from a flatbed trailer. The brake released, causing the aerial lift to strike another lift. The employee sustained a partially collapsed lung, rib fractures, and bleeding from the kidneys.

An employee was loading and unloading palletized material with a powered pallet jack. The pallet jack backed up and the employee's leg was crushed between the pallet jack and a bumper/trailer hitch, resulting in fractures to the right ankle and left tibia.

An employee was operating a forklift and went to grab a hold of the swinging arm to swing it into place. His hand was caught between the forklift and the swing arm, resulting in a middle finger amputation.

While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.

A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.

An employee's right middle fingertip was caught between a chain and a sprocket in a machine. The fingertip was amputated. The machine was unguarded at the time.

An employee had just finished unloading material from a truck. He was descending using the truck's built-in ladder when he missed the last rung, fell to the ground, and broke his hip and left wrist.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
