Federal OSHA recorded a severe workplace injury at Nestle, 300 W. Stevenson Rd, OTTAWA, ILLINOIS 61350 on — Fractures, affecting the multiple face locations.

An employee had just delivered product to a transfer station. He was on his way back into his truck when a dock plate gave way and he fell between the truck and the loading dock. A powered pallet jack fell with him and struck him in the face, fracturing his face and cheeks.

PUBLIX SUPERMARKETS, INC. STORE #00451

An employee was using a two-step step stool to stock product on shelves. The stool collapsed and the employee fell to the ground sustaining a fractured femur.

An employee was walking around and closing up the office for the day. As he was on a table closing the blinds, the table collapsed, causing the employee to fall. He sustained a left kneecap fracture.

Western Bays Constructors

Employees were demobilizing a microtunneling operation from inside of a 42-foot shaft. They were standing on a 3-foot high pump stand to disconnect the pump when the pump dropped, and the stand gave way. This caused the employees to slide and fall to the ground. The injured employee fell on his left side, landing on an oil can with his co-worker landing on top of him. The injured employee was hospitalized for a rib fracture.

E4A Solutions, LLC

An employee was on a wood platform surrounding a containment wall to work on a transformer. While inspecting the equipment, the wood platform collapsed, causing the employee to fall approximately 4 feet. The employee's shoulder was fractured and required surgery.

Pinnacle Contracting

An employee was standing on a baker's scaffold approximately 3 feet above finished floor to install equipment. While taking measurements, the locking pins came out on one end of the scaffold, causing the scaffold to collapse. The employee fell 3 feet and sustained a left ankle injury.

A&S Services Group

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Southeastern Freight Lines

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

Texas TransEastern

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

HOGAN TRANSPORT

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
