An employee was adjusting the pin lock on a chassis that had a container that was not sitting flush to it. The employee struck the pin multiple times with a hammer in the left hand and chisel in the right hand. On the last swing of the hammer, the chisel hit the pin and then slipped, sending the employee's right ring finger under the container as it fell flush to the chassis. The employee's right ring finger was crushed between the chassis and container, resulting in a fingertip amputation.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

He was resting at the top of the fixed ladder, with his upper body out of the tank hatch, when he collapsed and fell about 13 feet into the cargo tank. The employee sustained a fractured spine requiring surgery. The employee hospitalized.

An employee was using a pocket knife to cut a zip tie that was holding the slacked portion of line on a ratchet strap. The ratchet strap opened unexpectedly, causing the employee to lacerate an artery in his left forearm. The employee was hospitalized and required surgery.

An employee was walking across a catwalk on a vessel. The employee slipped and fell, striking their right side on the catwalk and suffered broken ribs on that side. The employee was hospitalized.

An employee was spray painting an area of a marine barge using a pressurized paint gun. Her glove became stuck to the gun. Her left hand came out of the glove and while trying to adjust the gun, paint was injected into her left middle finger.

An employee delivered a package. On her way back to her vehicle she was attacked by two dogs that came from around a corner. She was hospitalized with dog bites.

AHTNA Marine & Construction

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.
