An employee was inspecting a casted part that weighed 330 pounds. The part fell from a lift assist onto the employee, striking his upper right arm and head. He suffered burns to the upper right arm and was hospitalized.

On June 4, 2025, an employee was skimming molten aluminum when it dripped down from the skimming spoon onto the employee's left pant leg, burning the top of their left foot. The employee was hospitalized and required surgery.

An employee was loosening the legs of a metal desk on the production floor to adjust the height when the desk collapsed onto the employee's right hand, resulting in amputation of the thumb tip.

An employee was transferring molten aluminum into a sow (mold). Condensation dripped from a pipe located above the area into the sow as it was staged to fill. When the molten aluminum came into contact with the water, the molten aluminum reacted and was released into the air, contacting the employee. They sustained burns to their chest, neck, and bilateral upper extremities.

An employee was using a ladder to access a roof to install conduit pipes when the ladder slipped on patio tile and came out from under him, causing him to fall approximately 8 feet to the ground. The employee suffered a spinal fracture and brain bruising.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was working underneath a vehicle when the vehicle began to roll and a tire caught the employee's left ear. The employee sustained a laceration and partial amputation of their ear, requiring hospitalization and surgery to reattach the ear.

A car was being backed out of a bay so that another car could pull into it. The car's brakes did not respond, and the car reversed quickly. The injured employee approached the car to stop it, and it pinned his right leg against a lift. The employee was hospitalized.

An employee was walking down car garage stairs when he fell and suffered a broken right leg.

An employee was exiting the company shuttle van when his shoe became caught on the door of the vehicle, causing him to fall out of the van to the ground. The employee was hospitalized with a fractured hip that required surgery.

An employee was working to change the tires and brakes on a vehicle. He pulled the vehicle out of the bay. The brakes failed, and the car drifted backward into another bay where it pinned the injured employee against the arm of a vehicle lift. The injured employee sustained seven fractured ribs on his left side.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
