An employee was reducing pressure on an over-the-road tank truck for unloading purposes. Due to an over-pressurization event and/or a defect, a 3-inch stainless steel washout pipe nipple cap failed and struck the employee in the face. The employee was hospitalized with head injuries.

An employee was standing on top of a truck tanker investigating the process. A steam valve opened causing steam to come out of the oil tanker's dome lid. The employee sustained second-degree burns from steam and oil residue.

An employee slipped and fell while removing spots from the floor of a cargo tank barrel. The employee landed on a thermometer rod and was hospitalized with bruised ribs and a right side lung puncture.

An employee was stepping from the top of a cargo tank trailer onto the top rung of a ladder when they fell approximately 12 feet to the ground. The employee suffered multiple fractures to the right arm that required surgery.

While removing the valve fitting, sulfuric acid (in the form of a mist) sprayed out and contacted the employee's face and upper torso. The employee was hospitalized for chemical burns.

An employee was standing on top of a tanker truck with a water hose preparing to clean it when the water was turned on. The force of the water caused him to fall from the trailer, fracturing his right hip. The employee's lanyard was not secured at the time of the incident.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee had been loading a trailer. Their foot was caught in the trailer and/or trailer hitch, resulting in a severe laceration to a toe that required surgical amputation.

An employee climbed up on the tread of a crane to turn the batteries back on and was turning to step away when the battery lid opened back up. The battery lid caught his boot, causing him to lose his balance and fall to the tread. He then fell to the ground 36 inches below. The employee sustained a fractured femur head.

The injured employee was helping a co-worker pull wire through a conduit. The wire broke and the employee fell back and struck his right side against a tensioning coupler that was on the wall. The employee was hospitalized with two fractured ribs.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
