Final narrative
At about 11:00 p.m., on January 23, 2019, an employee was moving stock with a forklift. The employee dismounted the forklift and some tube stock slid off the forks, striking the employee's right foot and causing amputations to the big, first, and second toes. Steel toe boots were being worn at the time of the incident.
An employee was using a portable grinder to cut a metal stand when the grinder wheel shattered and caught his glove, pulling his hand into the grinder blade. He required surgery and hospitalization for a hand laceration.
An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.
