On September 20, 2022, an employee was installing wear pads under the newly installed mold carrier in a plastic injection press carrier. The employee used a crane with a nylon sling to slightly tilt the carrier so the wear pads could be placed. The sling failed, snapped, and struck the employee's left index finger, causing an amputation to the finger.

An employee was using straps to secure a load inside a transport trailer. The employee was standing on a stack of finished goods that was three pallets high. Each pallet was 16 inches high. He was walking from the stack of three pallets onto a stack of two pallets when he lost his balance and fell onto another stack of finished goods (that was two pallets high) and then landed on the floor of the trailer. The employee was hospitalized with fractured ribs.

An employee was unclogging a resin sifting machine. The rotary sifter's automatic system activated and the sifter blade struck the employee's fingertip, causing an avulsion. The fingertip was medically amputated.

An employee was helping to remove a roll of plastic from a line when the tail stock on the machine snapped back and caught the employee's right middle fingertip, resulting in a partial amputation.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
