Final narrative
An employee was working to remove the side forms from a 72" bulb tee line. He was unhooking the forms from a jack using lifting devices and steel pins. As he pulled the pin from the lifting device with his right hand, he attempted to push the pin with his left hand. His left index finger then entered the hole on the form brackets. The jack operator then lifted the device from the form, catching the employee's left index finger in the hole on the bracket. He suffered a left index fingertip amputation just below the first joint.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
