Final narrative
An employee was running a microwave bagging machine. A belt jammed; while the employee was fixing the jam, the employee's right index finger was amputated up to the first knuckle. The machine was guarded at the time.
An employee was troubleshooting a check weigher. The machine was turned off and the employee replaced the drive belt. When the machine started back up the employee's right index finger was caught on the moving belt and partially amputated. The employee was hospitalized and required surgery.
