An employee was troubleshooting a check weigher. The machine was turned off and the employee replaced the drive belt. When the machine started back up the employee's right index finger was caught on the moving belt and partially amputated. The employee was hospitalized and required surgery. The machine was not guarded at the time.

An employee was attempting to clear a jam in a dump tote when a solenoid energized and lowered a metal framework onto the employee resulting in severe crushing injuries. The equipment was not locked/tagged out at the time.

Another employee was operating a sit down forklift and struck the pallet jack pinning the injured employee's left leg between the pallet jack and the forklift resulting in an ankle injury.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.

An employee was cleaning a steam kettle; he had the floor grate lifted to access the drain area. He tripped on the open floor grate, striking his knee against the steam kettle's drain pipe and falling onto multiple pieces of kitchen equipment. He landed on the floor, where he struck his head. He was hospitalized with lacerations to his ear, which required surgery to repair.

An employee was outside a freight elevator, reaching in, when the elevator moved and crushed her right hand. She suffered a broken arm and wrist and was hospitalized, requiring muscle surgery.

On September 29, 2025, an employee was removing a pan of pasta from the stove when the pan tipped over. Hot water contacted her legs and feet, resulting in burns to both feet that required hospitalization.

An employee was walking through a passageway sideways when the edge of her shoe got caught and she fell onto her right hip, resulting in a broken femoral head.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.
