At about 11:00 a.m. on February 21, 2019, an employee was retrieving a steel channel beam from a stack. The employee pried two steel channel beams apart with a 12-inch pry bar to attach a lifting device. One beam slid off the other, and the employee's right ring fingertip was pinched between the beams and was amputated.

An employee was dumping trash into a compactor when he lost his balance and fell approximately 5 feet into the trash compactor. The compactor was not in use at the time. The employee was hospitalized with multiple fractures.

An employee was offloading containers from his truck using a deck that goes up and down. When the deck was up, he took a step and his foot went between the truck and the deck, resulting in a foot/ankle fracture.

An employee was using a hoist to lift large containers (420 pounds) of steel parts to be dipped into tanks. The employee set a container down on a support and his finger was caught between the container and the support, resulting in amputation just behind the first knuckle.

An employee was operating a riding mower. The employee's left hand became stuck between the mower's front left support bar and its reel. The employee suffered tendon damage to the middle three fingers.

An employee was guiding a motor into a container while a forklift was lowering it. The employee made contact with the motor, which amputated the top of his left thumb.

One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An employee was standing on the edge of the dumpster while pulling out cribbage. The employee fell to the ground, sustaining a fractured elbow and hip. The employee was hospitalized.

His left glove became entangled with the drill bit, which partially amputated his left thumb below the last knuckle and removed several inches of a tendon. The employee's left middle finger was also severely lacerated. He was hospitalized and received stitches.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
