At 10:45 a.m., on February 22, 2019, an employee was working at an extruder line, clearing accumulated plastic between the guillotine blade and pulley belts. The timed chopping cycle took place, amputating the employee's left index finger above the first knuckle. The employee was hospitalized, undergoing surgery.
