An employee was filling a motor fuel cylinder from a bobtail. When he opened the handle on the fill nozzle it popped off the filler valve and pushed him back into a rack where he struck his head on a cylinder cage. He sustained a laceration to the back left side of his head. While working to wrestle the hose to the ground, he sustained burns from liquid propane on the right side of his chest and right arm, and sustained minor burns on the left side.

An employee was making his weekly propane delivery. He connected a hose and refilled the first of two tanks. When he disconnected the hose, a fire occurred and the employee received second degree burns to both arms and the left side of his face.

An employee was performing oversight for the welding of a pipeline launcher door on a construction project. Two welders were performing arc welding, one on the interior diameter of the pipeline launcher door, and the other on the outside diameter of the launcher door when a flash fire occurred. The injured employee sustained first- and second-degree burns to the face, neck, back, and hands.

While on a scissor lift, an employee was grinding a steel pipe. Sparks from the grinder ignited some nearby rags that had been used to apply coating treatments to the pipe. This fire subsequently ignited the employee's clothing. The employee sustained burns to approximately 46% of their body and was hospitalized.

An employee was working in an area where another company was also working. The other company's employees were using an electric hydraulic pump to remove the bolts of the frac valve. The wiring of the torque wrench and pump unit were then charred and a flash fire occurred. The employee sustained second-degree burns to their face and neck.

On October 17, 2023, an employee was at a sink while using a solvent to unclog a vacuum trap dip tube that contained solids from pinacolborane distillation. During this process, the sink caught on fire and the solvent bottle containing tert-butyl methyl ether fell into the burning sink. The employee attempted to remove the solvent bottle which also caught on fire and suffered burns to their neck and left hand.

An employee was under a railcar cleaning up dust with a vacuum hose. The hose caused a spark that caused a flash fire that ignited the employee's clothing. The employee sustained second- and third-degree burns over 83% of his body.

An employee was connecting a hose to fill a propane cylinder. The flow of propane was activated when the filler nozzle was partially connected to the cylinder. The nozzle detached from the cylinder valve, causing expanding liquid propane to release through the nozzle. This caused the employee to lose his balance, spin, become entangled in the hose, and fall. He struck his head on the ground and suffered fractures to the temple and cheekbones, a laceration on the forehead, and a brain bleed. The employee was hospitalized.

An employee was pumping liquid propane from a propane delivery truck to a storage tank. After the transfer was completed, the employee closed the valves and disconnected the transfer hose. During the transfer, the liquid propane auto-refrigerated the riser pipe of the storage tank and that delayed the vaporization of the propane. Upon reaching a higher temperature, liquified propane began rapidly expanding. Liquid propane then discharged from the pipe and contacted the employee, causing cold burns. The employee was hospitalized for second-degree burns to their torso and arms.

An employee was delivering propane to a customer when he heard a pop in his lower back. The employee was hospitalized with a back injury.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
