One of the stamping arms was not operating properly, and the employee was on top of the mold chain conveyor trying to fix the stamping arm. The employee was walking up the mold chain conveyor when the employee s foot became caught in the mold against the slide and the machine began to pull the employee down with the chain. The employee's right foot and ankle were smashed, and large amounts of swelling occurred. The conveyor was moving at the time of the incident.
