An employee was lifting a bag of soiled laundry when they began to experience pain in their abdomen, resulting in hospitalization due to a hernia.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was working on a washer. He was standing behind the machine and walked through the hydraulic absorbent pad. As he walked around the machine he slipped and put his hand down to brace himself. The employee contacted the machine's belt and pulley, resulting in a hand injury. The machine was not guarded at the time of the incident.

An employee was working on a scissor lift near a hook conveyor belt system when a cord from the conveyor belt got stuck on a bolt of the scissor lift causing the lift to fall over. The employee fell from a height of 12 feet to the concrete floor below and sustained a fractured pelvis, broken ribs, an abrasion on a lung, and chipped vertebrae.

On 12/6/2017, an employee was attempting to remove a linen sheet that was caught in a sheet folder ironer system when her hand was pulled into the machine and fractured.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

The upper presser closed down on the employee's arm resulting in hospitalization with burns.

An employee was fixing a steam leak from a boiler. He used rope to secure the manhole cover while venting residual steam from the manhole opening. When he released tension on the rope, the manhole cover dropped into the tank and splashed hot water onto the employee. He sustained first- and second-degree burns to his right arm and first-degree burns to his upper chest.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
