On May 4, 2018, an employee started to build a scaffold when the vertical scaffold leg fell and hit the cross braces. The employee's left ring finger was caught between the cross braces and the vertical scaffold leg. His left ring fingertip was amputated (no bone loss).

An employee was receiving scaffold materials up to the 21st floor via an engineered opening in the scaffold flooring. The employee fell approximately 14 feet through a gap in the scaffold flooring. The employee struck his head in the fall and was hospitalized. The employee was not tied off at the time.

An employee slipped on a scaffold ladder rung, lost his grip, and fell approximately 14 feet to the ground.

An employee was walking back to her workstation. A forklift reversed a short distance, then completed a U-turn and struck her, running over her lower extremities. She was hospitalized with a broken ankle.

An employee was training on a jockey truck. Upon stepping out of the truck, the employee was struck by a rubber tire gantry crane, resulting in amputations of the right leg above the knee and left leg below the knee.

An employee was standing at the rear of a double pallet jack. Another double jack approached, and the employee's lower leg was caught between the two pieces of equipment. He suffered a broken tibia and fibula.

An employee was unloading large material bags on a loading dock. A forklift tine pinned the employee's foot against a pallet, causing the employee to fall and suffer a broken ankle.

An employee was pushing a container when they were struck by a forklift. The employee sustained a left leg fracture.

An employee was using a 10-foot step ladder to install roller shades. The employee fell from the third rung from the top, landing on the ground and suffering back and head injuries. The employee was hospitalized.

An employee was climbing a ladder to access a home attic. The employee fell from the ladder to the ground, suffering a broken right elbow.

An employee was cutting a cast iron pipe when the pipe fell onto his right hand, resulting in amputation of the thumb. The employee was hospitalized.

While on a scaffold, an employee was transporting heated sealant material in a bucket. The employee tripped and fell, causing the heated material to splash onto his gloved hands. The employee sustained second-degree burns to both hands and his right wrist.

Three employees were pulling tension cable (similar to rebar) through the walls of a condo using a motor system. Two employees were needed to hold the cable while the third employee drilled the mounting locations. When the third employee activated the drill, the cables twisted. They caught the right index finger of one of the other two employees, amputating his fingertip.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
