An employee was cleaning out the rear of the company van and when he closed the rear door, his right thumb was caught in the gap between the hinges. The tip of the thumb was partially amputated.

An employee was being lifted in a powered industrial truck cage to conduct inventory. The cage fell to the ground when it reached the third tier of the racking system, which was about 25 feet high. The employee hit the ground, sustaining facial fractures and bruised ribs and lungs.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

XPO, Inc.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

An employee was attempting to remove a tarp from a flatbed trailer when the trailer handle struck the employee in the chest, resulting in two broken ribs and a punctured lung.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

Gold Star Feed & Grain, LLC

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was standing with one foot on the edge of a lift, leaning in to read the part number on a carton. He stepped back, began to fall, and grabbed the rack support to stop himself from falling. A hole in the rack caught his right little finger and amputated it.

An employee tripped over a bathmat and fell on the floor. The employee sustained a spiral fracture to his right leg requiring hospitalization.

An employee climbed to the 6-foot level of a ladder on a deck to remove screws and trim boards when the ladder slipped on the deck. He fell to the side and then jumped to the floor off the deck, landing on his feet before collapsing. He fractured his right tibia and injured his left heel, requiring surgery.

An employee was sitting at her work station when she got up and tripped over a purse strap that was on the floor. She was hospitalized with a broken right femur.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
