Four employees were repairing/installing guardrails when they were struck by car. All four employees were hospitalized. One employee suffered a leg amputation. A second employee sustained fractures and internal injuries.

An employee had been leveling out a rock base in the storage yard. He tripped and fell on broken concrete, sustaining a knee laceration, torn tendon, and a punctured knee cap. The employee was hospitalized.

An employee was using a chainsaw to trim a tree on the shoulder of the highway. The employee either lost his footing or was hit by a branch and fell from the tree, landing on one side. The chainsaw landed on his other side. He sustained a compound fracture to one leg.

An employee was taking a break from laying sod. When he went to get up, he fell backward and needed assistance to get up. He suffered from dehydration.

An employee was sitting in a vehicle and was ready to set rail on a railroad crossing when a train struck the vehicle with the employee inside. The employee suffered fractured ribs.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was helping to set up a mulch blowing truck when they were struck by an oncoming vehicle. The employee was hospitalized with a fractured knee, a concussion, and a head laceration; they also received stitches on their arm.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was mowing a residential lawn at a client's house. The mower slid on the land and tipped over onto him. The employee was hospitalized with a fractured vertebra in the neck and partial paralysis.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
