An employee was doing maintenance on a semi trailer from a rolling ladder. The ladder broke, causing the employee to fall and strike the ground. The ladder landed on top of the employee. He suffered a broken left hand and two broken bones in his right foot.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
