Employee 1 was operating a forklift in reverse and collided with another forklift, operated by Employee 2, that was stopped in an aisle. The forks of the moving forklift were raised and penetrated Employee 2's right leg upon impact. Employee 2 was hospitalized with lacerations and a fractured tibia.

An employee was riding on a powered pallet jack near a sewer drain. The pallet jack hit a dip near the sewer drain, and the employee s left foot hit a clamp truck parked in a charging station. He suffered a cut to the big toe, as well as fractures in the toe and the foot.
