An employee was walking to the gangway of a vessel. Their left hand was caught by the roller track of a hatch cover on the vessel resulting in partial amputations to the index and middle fingers and a laceration to the same area.

Two employees were discussing an operational assignment when a reversing forklift ran over the injured employee's right foot, resulting in a fractured right ankle.

The injured employee was removing a piece of 4" x 4" lumber from steel slab cargo in the yard area while another employee was operating a heavy-duty powered industrial truck (PIT) to lift a steel slab with magnets using a reach stacker. The lift was reversing with the cargo hoisted when the rear tire of the PIT hit a pothole and caused the slab to swing back and forth. As a result, the steel slab struck the injured employee in his abdominal area. The employee suffered a pelvic fracture, fractures to the L4 and L5 vertebrae, and a possible bladder contusion.

A load of cargo from a vessel was being landed on the dock. The injured employee walked towards the load in order to unhook the rigging, when one of the 4-ton round steel bars rolled and came to rest on his left foot. The employee was hospitalized with a fractured left ankle.

While working to free the handle of the tension binder from the deck plate using the handle of another binder, the binder suddenly released and struck the employee on the left side of his face and on the right thumb. The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

An employee was standing on the outside edge of a hopper barge while watching a skid steer push ore for an excavator (with a clamshell bucket) to unload. The excavator was on a work barge. The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.
