The employee jostled the hitch pin while continuing to let the jack stand down. The hitch slipped while the employee's fingertip was in the safety pin hole, causing a fingertip amputation.

An employee was attempting to clear a product jam on a breader machine's conveyor belt when he suffered lacerations to his first left-hand finger.
