HELP AT HOME, INC.

At about 2:00 p.m. on March 20, 2019, an employee was walking down the steps of a client's home when she fell down six steps. She suffered bruised ribs, a nasal fracture, a possible bruised frontal lobe of the brain, and injuries to the left knee and ankle. She was hospitalized.

An employee was cleaning a client's residence and was struck by a folding chair that fell from a height or possibly tripped over a chair and fell. The employee suffered three fractured ribs and a lacerated spleen.

An employee suffered a ruptured/fractured vertebrae while assisting a patient.

During an altercation, an employee was stabbed in the stomach/chest area.

Two employees were involved in an altercation in a parking lot. One of them was stabbed with a knife on the right side of his abdomen. He was hospitalized.

A security guard was working in a parking garage when an unauthorized person entered the garage. The security guard requested that the individual leave and they were stabbed with a knife. The employee was hospitalized.

An employee stabbed the injured employee with a knife in the breakroom during an altercation. The injured employee sustained stab wounds to their chest and abdomen.

An employee was walking from the receiving area to the grocery section. A customer was waiting for the employee around a corner and stabbed the employee multiple times with a sharp object. The employee was stabbed in the back, abdomen, and left side.

An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.

An employee was seated in the living room of a patient's residence, waiting for the patient to arrive home from school. The school bus driver honked the horn, signaling that the patient was ready to be assisted off the bus. The employee stood up and then tripped and fell over a rug and possibly the edge of an ottoman. The employee was hospitalized with a fractured right humerus at the shoulder.

An employee was assisting a client when the client fell on top of the employee. The employee's hip was fractured.

An employee was retrieving their purse and keys when they tripped over a cord and fell to the ground, resulting in a broken arm and five broken ribs.

An employee was carrying two bags of laundry down some steps. She missed a step and tumbled forward, suffering a neck injury. She was hospitalized.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
