Four employees were pushing a roll of paper on the paper machine winder deck to clear a conveyor for two defective rolls. They lost control of the roll and it struck one employee resulting in a fractured pelvis that required surgery.

The employees were bringing the paper machine back online. The waste paper stock went into the overflow chest to be returned to pulp. An employee went out to observe the overflow from the chest in the street to make sure the sewer drain did not clog. As the employee was standing by the sewer drain, the condensate tank released to the same area. The condensate, mixed with the product from the overflow chest, caused the product level at the curb to rise over and into the employee's boots burning his feet, ankles, and lower legs.

Employees were moving calendar rolls around in the roll storage area of a deep well. The injured employee was walking between a suspended roll and a roll on the ground when a crane operator changed the direction of travel to avoid striking an adjacent roll. This change of direction caused the suspended roll to pin the employee between the suspended roll and another roll, fracturing his pelvis.

An employee threading a calendar stack in a paper machine attempted to clear wrap that had wrapped around the stack. The employee's left hand got caught in a nip point, amputating the pinky finger and partially amputating the ring finger.

An employee had just finished repairing the chip mill saw. While walking on the metal decking, he tripped on saw dust, hit the decking, and fractured his left femur bone.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

An employee was staking tomato plants on a farm. The employee began to feel sick and collapsed, suffering from dehydration and heat shock.

An agent was participating in SWAT team selection and was performing various physical fitness skills including running and exercise intervals. The agent experienced dehydration and a muscular injury that required hospitalization.

An employee was traveling back from work when they became ill and experienced dizziness due to heat exhaustion.

On September 26, 2023, an employee was delivering packages when he began to feel ill with a pain in his side. He was hospitalized for heat exhaustion and dehydration.

Tools and parts were being lowered to the ground from an elevated platform in a 5-gallon bucket. The rope being used to lower the bucket failed; the tools and parts fell and lacerated the left forearm and upper left leg of an employee on the ground.

An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.

An employee was working to clear a jam from a paper napkin folder. The ram that advanced the napkins pushed the employee's right arm into the side of the machine, causing several lacerations and a hairline fracture.

An employee was troubleshooting overflowing ink on a printer-slotter machine when their right ring fingertip was caught in a guarded roller, resulting in a partial amputation.

An employee was disconnecting wires in preparation to swap out a refiner motor when they contacted energized equipment and sustained an electrical shock. The employee was hospitalized.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
