An employee was putting product away in a warehouse. While walking backwards he struck a stack of pallets, fell and sustained a broken ankle.

An employee was performing preventative maintenance on an exhaust fan on the roof of the engine room. When the employee removed the lid to the exhaust fan, his hand slipped and contacted the belt inside the fan. The tip of his left ring finger was amputated.
