An employee was operating an enclosed pallet jack and stopped to scan a pallet. As the employee reached out to scan the pallet, the cord of the scanner hit the throttle of the pallet jack. The pallet jack lunged forward and pinched the employee's right leg against a doorway protection pole resulting in a lower leg fracture.
An employee was performing preventative maintenance on an exhaust fan on the roof of the engine room. When the employee removed the lid to the exhaust fan, his hand slipped and contacted the belt inside the fan. The tip of his left ring finger was amputated.
