An employee was assisting with an operation that was pulling in an underground wire. The employee was guiding a rope through a capstan when their gloved right hand was pulled into the capstan for approximately 1/3 of a rotation. The employee's right index fingertip was crushed and surgically amputated to the first joint.

An employee was climbing a pole during a training exercise. He climbed above a secondary line, reconnected his fall protection device, put his weight on it, and it held. The employee fell when he tried to put his weight to the device again. The employee suffered a broken right heel and a vertebra fracture in his lower back.

An employee was between the trough and casting pit, cleaning a filter box at the end of a cast. His right foot slipped off the lid of the cast pan and went into an opening where molten aluminum goes into a trough. The employee contacted molten aluminum and sustained third degree burns to his right leg, ankle, and foot.

Lewis Tree Service, Inc.

An employee was aloft in a tree cutting tree limbs. He was using a rope system and while repositioning, he slipped on moss that was on the tree bark and struck his knee awkwardly on the tree. The employee sustained a fractured kneecap and was hospitalized.

An employee lost their balance and slipped. Their hand contacted a turning rotary valve and their middle fingertip was amputated.

An employee was leaning against a fry station when his legs began to slip out from under him. He went to break his fall when his right arm entered the cooking oil. He suffered second- and third-degree burns to the arm.

An employee was unlashing (loosening chains) military vehicles on rail cars prior to offloading. As he stepped off the railcar and onto the ground, he twisted his left ankle in loose gravel alongside the rail track. He required surgery for a dislocated ankle.

An employee was performing troubleshooting and maintenance on one of the generators that feed the RF line for glue-laminated timber (glulam) beam production. He was reattaching the outer panel of the generator. A circuit was completed and the employee was electrocuted by the generator and sustained burns. The employee was hospitalized.

An employee was utilizing a utility task vehicle (UTV) to patrol a 138 kV transmission line in a right-of-way. The UTV drove into a washout hole that was approximately 6 feet deep and obstructed by vegetation. The employee sustained a dislocated shoulder, a fractured left humerus, back pain, and a strained ankle. He was hospitalized.

Central Maine Power Company

A crew of employees were working to replace a utility pole that was broken by a motor vehicle accident. As the crew was removing equipment from the broken pole, part of the pole broke, fell on the injured employee, and rolled over him. The employee sustained two broken shoulders, multiple broken ribs, and a damaged lung that required hospitalization and surgery.

Ameren Illinois - Sparta Operating Center

An Ameren lineman was making secondary voltage connections in a pad-mounted residential feed transformer. The lineman contacted an energized lead bushing, sustained an electrical shock, and was hospitalized.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below.
