An employee was walking on the jobsite, tripped over an elevated manhole and fell to the ground. The employee sustained a broken femur.

An employee was tying down a tractor on a flatbed trailer with tie down chains. He then stepped back off the trailer to the ground, and felt pain in his left hip. The employee sustained a torn hip flexor.

An employee was unloading tools. As they stepped off a lift gate of a box truck to the trailer, their left knee buckled, causing the right knee to buckle as well. The employee sustained a right knee fracture and a ruptured quadriceps tendon on the left and right leg.

An employee stepped down from a piece of fracking equipment into mud. He twisted his ankle and fractured it.

An employee was exiting his truck when he twisted his ankle on its ramp. He suffered a sprain.

An employee was getting into a semi truck when his back popped and he fell.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
