An employee was walking to a break room when his right foot was caught under a parked rider pallet jack, causing him to fall. He suffered a broken left hip and was hospitalized.

A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

A patient kicked a police officer in the leg, rupturing their patellar tendon. The police officer required hospitalization and surgery.

A cashier was hit in the face by a customer. The employee sustained injuries to his head and face.

An employee was making patient rounds at the hospital and stopped to talk with the charge nurse in front of the nurse's station. A patient reached over the nurse's station, grabbed a metal three-hole paper punch from over the desk, and proceeded to attack the employee with it. The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was on the floor of an aisle wrapping a pallet when an electric pallet jack struck him, resulting in fractured ankles.

An employee was removing a rug entangled in the rear tire of a powered industrial truck (PIT). The PIT was raised and supported by a fork truck jack. The PIT shifted/fell and struck the employee's hand. The employee's index fingertip was amputated and the employee was hospitalized.

On May 14, 2025, at approximately 6:30 AM, a driver was making a delivery at a customer s site when their hand was crushed in a roll-up door. The employee's finger was fractured in three locations and required surgical amputation.

An employee was operating a tractor and utilizing a 2.5-foot kinetic rope and a 1-inch soft shackle to pull another tractor that was stuck in the mud. An additional 30-foot kinetic rope with a soft shackle was hooked up to provide distance from the soft soil. While pulling, the loop end of the terminal shackle on the stuck tractor frayed and released the rope, causing it to recoil back at the pulling tractor. The rope(s) struck the back of the pull tractor, broke the rear window, and struck the employee. The employee was pushed forward into the steering wheel and sustained severe lacerations to the head and a collapsed lung.

An employee was staging products in the back of their box truck to be unloaded. He slipped on a surface in the truck that was wet from rain, and fell out of the back of the truck onto the concrete ground. The employee was hospitalized with a fractured left humerus and required surgery.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
