An employee was backing an electric industrial carrier into a dolly when their fingers were caught between the vehicle and the bar on the line resulting in amputation of the fingertips on their right middle and ring fingers.

A forklift struck an overhead fan and some racking. The fan fell onto an employee working on the dock, who suffered lacerations and ruptures to the spleen and liver. The employee was hospitalized.

An employee was working with an electric socket gun while training another employee. The trigger on the socket gun was activated and the employee's glove became caught, resulting in an amputation of the right index finger below the knuckle.

On September 18, 2025, an employee was rolling a stack of two steel racks containing automobile bumpers when the top rack dropped, pinching their right little finger. The employee suffered a partial amputation.

An employee spent about 4 1/2 hours working outdoors, servicing a boom lift on a truck. He was hospitalized with heat stress.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.
