An employee was drilling holes in a road. The drill caught his left ring finger and partially amputated it.

An employee was unloading liquid asphalt cement from a semi-tanker. He left to retrieve a new gasket for the tanker's hose. The valve was opened while he was gone, and when he returned and removed the hose, 300-degree asphalt cement sprayed out of the connection and struck him. He suffered second-degree burns to his left and right forearm, left thigh, lower stomach, and right shin toward the ankle.

An employee was cleaning cement off an end dump trailer's tailgate latch. The tailgate closed, pinching the employee's left ring finger between the gate pin and the latch and amputating the fingertip just above the nailbed.

Employee had just finished unloading a truck into the paving machine. He had his hand on the gate of the truck. The truck operator latched the gate from the cab, crushing the pinky finger on the employee's right hand.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

An employee was staking tomato plants on a farm. The employee began to feel sick and collapsed, suffering from dehydration and heat shock.

An agent was participating in SWAT team selection and was performing various physical fitness skills including running and exercise intervals. The agent experienced dehydration and a muscular injury that required hospitalization.

On September 26, 2023, an employee was delivering packages when he began to feel ill with a pain in his side. He was hospitalized for heat exhaustion and dehydration.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
