An employee was unjamming a trimmer machine and suffered a severe laceration and fracture to the right arm when one of the interlocks failed. The machine was guarded and interlocked at the time.

An employee was making an adjustment to a blow mold machine when a tool slipped from his hand and caused him to lose his balance. His left hand then came in contact with a mold part, amputating his index and middle fingers at the knuckle and his ring fingertip.

An employee fractured the right foot while changing the battery in a stand-up electric forklift.

An employee was working at a filler station. A bottle jammed in the machine. The employee reached into the machine to remove the jam and suffered a partial amputation of the right index finger.

On February 20, 2015, at approximately 6:30 a.m., a maintenance employee was using a 12-inch pedestal grinder to grind spurs off a small piece of metal he was holding in his hand at a 45-degree angle. The end of the piece of metal got caught between the grinding wheel and the work rest, causing the piece part to become lodged in a fixed vertical position against the grinding wheel and catching the employee's left thumb between the piece part and the grinding wheel. The employee was treated in the emergency room immediately following the accident with a fractured and lacerated thumb. Upon subsequent referral to a hand surgeon, the diagnosis was expanded to include partial amputation of the thumb.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was performing rear brake shoe service on a truck. The employee was using a prybar to remove a spring when the spring slipped from the S-cam and amputated his left index fingertip.

An employee was unjamming a case that was stuck in a case washer machine when their pants became entangled in a chain, causing them to be sprayed with hot water from the machine. They employee suffered burns from the hot water.

An employee was cleaning out a butter fat tank. The employee slipped and fell, suffering a broken right leg.

An employee was clearing a cup jam on a cup conveyor when their left hand was caught between the conveyor and an idler roller located on another conveyor line directly above the jam. The employee received an abrasion and friction burn to the hand that required hospitalization.

An employee was performing routine maintenance on an air valve when their right ring finger became caught in the valve. The employee sustained an amputation to the fingertip.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
