While doing housekeeping in the early morning, an employee tripped on an HDPE pipe and fell, breaking his ankle.
An employee was testing a 16-inch pipe with a test ball when the test ball flew out of the pipe causing the employee to fly back, landing on his right arm. The employee was hospitalized with a broken arm.
An employee stepped onto an unsupported cantilevered platform. The platform gave way and the employee fell approximately 7 to 8 feet into a water retention pit below, suffering a head laceration.
