On August 20, 2019, an employee was adjusting the forks on a powered industrial truck when a second employee drove down an adjacent row and struck/pushed returnable metal frame packaging toward the first employee. The first employee was pinned between the packaging and the mast of his powered industrial truck, suffering fractured ribs, a lacerated kidney, and a lung contusion.
