Final narrative
An employee was guiding a chiller head as it was being hoisted by a chain hoist. A strap that was used to hang the chain hoist snapped, causing the chain hoist and the chiller head to fall. As it was falling the chiller head hit a section of steam piping and veered off towards the employee, landing on the bridge of his right foot. Four metatarsals were each fractured in two places, requiring hospitalization.

On 2/5/16 at approximately 9:00 a.m., an employee was loading four-inch black pipe onto a truck rack. The employee was securing the pipe into straps and a pipe rolled back onto his hands. The employee was wearing leather work gloves. The pipe crushed his right middle finger on his right hand resulting in a hairline fracture and laceration requiring sutures. The employee was later hospitalized for treatment of an infection.
An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.
