Final narrative
An employee was re-stacking boxes that had fallen. A double-stack of pallets that was behind the employee fell and landed on the employee resulting in a fractured right ankle.
While attending the slab line, an employee observed that dough was accumulating on the plastic blades. He proceeded to clean the blades. His left index finger came into contact with an adjacent blade that moves laterally (left to right), resulting in a partial amputation.
An employee was assisting coworkers in moving equipment. Their finger was pinched when placing the equipment down on a moving cart, resulting in a fingertip amputation.
A piece of film jammed in a machine. An employee was trying to remove it when a knife (part of the machine) came down and lacerated the fingers on her left hand.
At about 2:00 p.m. on August 2, 2021, an employee was making batches of popcorn when the kernel dispensing tube cap came off. The employee was putting a lid/cap on the tube system when the auger (spiral rod) grabbed his lab coat and pulled him in. The auger penetrated the employee's chest and exited his back. He was hospitalized.
