Federal OSHA recorded a severe workplace injury at Tire Discounters, Inc. , 3650 E Kemper Road, CINCINNATI, OHIO 45242 on — Fractures, affecting the multiple foot (feet) locations.

An employee was in a warehouse that had three 6-foot high tiers of tires stacked on top of each other. The employee had climbed on top of a picker, stepped on the second tier tires, and was reaching for the third tier when the tire he was standing on slipped. The employee and tire then fell to the concrete floor, breaking his foot and toe.

An employee was replacing a tire on a wheelbarrow when the tire exploded. The employee sustained fractures to his right hand and an abrasion to the right side of his face. The employee was hospitalized and required surgery on his right hand.

The employee was on top of a rail car to level the grain that was deposited into the car when they fell 13-15 feet to the ground. The employee sustained a right leg fracture.

An employee was on an extension ladder installing a satellite dish on a roof when they fell approximately 12-15 feet to the ground. The employee sustained fractures.

An employee was setting a 5-foot pry bar into a precast concrete slab and stood on the end of the pry bar to move the slab into place. The pry bar slipped out causing the employee to fall backward 15.5 feet to the dirt ground. The employee was hospitalized with a broken leg.

An employee was on a step ladder to measure and mark a crane rail for modification. The employee fell approximately 12 to 15 feet to the ground, resulting in a fractured right hip.

An employee was on a trailer securing a load of logs when they fell approximately 9-12 feet to the ground. The employee sustained brain hemorrhaging and lacerations to the thigh and above the eye. The employee was hospitalized.

An employee was rolling a rear tire for a tractor to a customer's building. The tire started to tip and the employee went to stop it by stepping into the middle of the tire. The edge of the tire struck his head and knocked him to the ground and the tire landed on his head. The employee was hospitalized with a fractured and dislocated left hip that required surgery and a scalp laceration.

Goodyear Tire & Rubber Company

An employee was pulling a metal skid off a stack to place the skid on the floor. The weight of the skid shifted and the edge caught the employee's right middle finger, resulting in a partial amputation.

Gateway Tire of Texas

An employee was operating an order picker in a warehouse. While backing down an aisle, the employee struck a 60-inch-by-60-inch pallet being lifted by another order picker. The employee suffered a tear to the cartilage surrounding their hip.

At about 1:25 p.m. on August 7, 2024, a temporary employee was operating a foam ring cutter when its pin broke, causing the head to fall and hit his left hand. He suffered a partial amputation to the ring finger, as well as a laceration and nail damage to the middle finger.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
