On September 23, 2019, a blow mold assembler suffered a crushing injury to her left middle finger in the nail area, resulting in a partial finger amputation.
An employee was adjusting a capper. The drive chain and sprocket caught and injured the employee's right hand, causing the amputation of three fingers.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
