Three employees were rolling a portable conveyor. One of its wheels fell into a drain, causing the conveyor to tip and fall onto the injured employee. The employee suffered injuries to the shoulders and legs and was hospitalized.

A driver was opening a trailer door when a gust of wind blew the trailer door open, hitting the driver in the face and knocking him to the ground. The employee suffered a hip injury.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was driving a utility cart into a garage area. The vehicle struck the bottom of the garage door as it was opening and the employee sustained fractured ribs.

An employee was performing retails tasks including bending over and lifting 35-pound cases of product. After lifting about 25 boxes he felt back pain, resulting in severe nerve root compression at the base of the spinal cord due to a disc herniation. He required hospitalization and surgery.

An employee was cleaning the kitchen at the end of a work shift. As the employee and a co-worker lifted a kettle containing hot water from a hot stove to empty the water, the injured employee slipped and fell. The kettle was dropped, causing water to splash onto their hands and legs. The employee sustained third-degree burns to their hands and quadriceps.
