An employee was closing the roll-up door on the back of a delivery truck. The door slammed on his right hand, causing a partial amputation to the little finger.

An employee was picking cases in a four-deep pallet location with two pallets left in it. As the employee was taking two cases off one of the pallets, their weight pulled the employee off the second level of the modular line. The employee landed on the floor and suffered a broken right wrist.

An employee was attempting to free a pallet stuck on its roller system. The pallet would not move so the employee braced himself against the mezzanine guardrail system and pushed. The weld on the steel upright broke and the employee fell to the floor, breaking both legs and suffering one compound fracture. He was hospitalized and had surgery.

An employee was operating a reach lift and caught his foot between the lift and a piece of equipment.

An employee was picking product while standing on a hoisted pallet. The employee then fell from the pallet to a concrete slab, requiring hospitalization.

An employee was picking up a car from a fenced-in area when they became locked inside. The employee climbed the 7-foot fence, jumped off onto the concrete surface, and sustained bilateral ankle fractures.

An employee was descending an extension ladder that was set up on a cable strand at approximately 18 feet. When the employee was about halfway down, the ladder started to shift to the left and the employee jumped off the ladder to the right. The employee landed on a concrete sidewalk and their right ankle was fractured.

An employee was practicing an acrobatic act and jumping from one swing to another when she fell approximately 10 feet to the wood floor below, resulting in a lower lumbar fracture. Fall mats were in place at the time of the incident, but the employee fell outside of the mats.

A driver was inspecting an empty container for damage. While the driver was inside the container, a stacker operator started lifting the container, which was still attached to the chassis with four twist locks. The driver, upon feeling movement, jumped approximately 6 feet out of the container onto the concrete ground and broke his heel in two places.

An employee climbed an 8-foot fence. When the employee jumped down, their right tibia and left ankle were broken.

An employee was working on the dock unloading a truck when he became dizzy and ill. The employee was hospitalized with heat exhaustion.

An employee was riding a pallet jack when it slid on a wet spot. When he went to jump off, his right foot was caught between the machine and the warehouse curb. The employee was hospitalized with a fracture to his right heel.

An employee slipped on the wet store floor and fell, resulting in a fractured right leg.

An employee was climbing down a ladder when he slipped and fell to the ground. The employee sustained broken ribs and a lacerated spleen.

On November 6, 2024, an employee was loading a portable hand truck with cases of beverages into the back of his truck. While exiting the truck, he slipped and fell on the ramp and the hand truck struck his pelvis, causing lacerations and bruises. The employee was hospitalized.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was assembling a baker type scaffold. He was beginning to transverse down the scaffold, approximately six feet off the ground, when the scaffold tipped over and he fell. The employee was hospitalized with a fractured femur.

An employee was stocking and ordering the meat case on the sales floor when he slipped and fell due to water on the floor. The employee was hospitalized with a fractured ankle and required surgery.

An employee was working on an exhaust fan on the roof when he tripped on a wire and fell to the ground. The employee sustained scrapes on his elbow, and a fractured right knee with a torn ACL. The employee was hospitalized.

An employee was climbing a scaffold when he slipped and fell 10 to 15 feet to the ground. The employee was hospitalized with a fractured ankle.
