An employee was on top of a 30-yard roll-off box that was loaded with pallets, trying to remove the pallets from the sides so that he could tarp the box. As he tried to dislodge a pallet that was sticking up out of the top of the box, it suddenly came free. He stumbled backward, then fell off the box to the ground, suffering a cut on the left side of the head and a concussion.

An employee was cleaning a dock-locking mechanism on a solid waste container on the back of a truck. The latching mechanism was engaged, catching the employee's right glove and partially amputating the little finger at the second knuckle.

An employee was talking to a forklift operator when the forklift backed into the employee, resulting in a fractured ankle.

An employee was standing at the rear of a garbage truck loading trash when the front passenger tire struck his ankle/foot. The employee was knocked down and the tire rolled over him, resulting in fractures to his ribs and ankle.

An employee was standing on the street, preparing to leave a job site. A pickup truck with a wood chipper trailer was backing up and pinned the employee against the front of a skid steer. The employee suffered multiple fractures to the right femur.

A temporary employee was removing a traffic cone on a street when a delivery truck backed into him. He fell and suffered a back injury.

On October 8, 2021, an employee was on the back side of a truck, giving the driver directions to back up the truck to get close to two garbage cans. The truck struck him, causing fractures to his pelvic bone and sacrum. He was hospitalized.

An employee was operating the compaction levers on a truck when a motor vehicle crashed into the back of the stopped truck. The employee sustained fractured ribs and contusions.

A maintenance employee was moving a forklift from the loading dock when the forklift fell off the dock. The employee was struck by the overhead portion of the roll bar, injuring his right knee. He was hospitalized and underwent surgery.

An employee was collecting garbage when they suffered muscle cramping and were hospitalized due to dehydration.

An employee was climbing a truck-mounted ladder when the ladder broke, resulting in hospitalization with a fractured left leg.

An employee was standing on the driver side rear step of a trash truck while traveling down a road to collect residential trash. The employee went to signal the driver and slipped off the step while the truck was traveling 8-9 miles per hour. He fell 18 inches to the ground and was hospitalized with a fractured ankle that required surgery.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
