An employee was loading a trailer of automotive parts when the load caught the top of the trailer. He jumped up to clear a wedged item. When the item came down, the load collapsed onto the employee, resulting in a fractured pelvis.

An employee was performing maintenance on an a conveyor system. He was working to loosen the chain that moves the belt when his right index fingertip was caught by the chain and amputated.

An employee was walking on the dock when a forklift ran over their right foot. Their foot was crushed. The employee was taken hospitalized and required surgery.

An employee was cutting corner boards off a pallet with a box cutter. The box cutter lacerated his arm.

An employee was collecting pallets in the freezer when his foot slipped on condensation on the platform of the pallet jack and struck a post, resulting in a foot fracture.

An employee was standing at the rear of a garbage truck loading trash when the front passenger tire struck his ankle/foot. The employee was knocked down and the tire rolled over him, resulting in fractures to his ribs and ankle.

An employee was standing on the street, preparing to leave a job site. A pickup truck with a wood chipper trailer was backing up and pinned the employee against the front of a skid steer. The employee suffered multiple fractures to the right femur.

A temporary employee was removing a traffic cone on a street when a delivery truck backed into him. He fell and suffered a back injury.

On October 8, 2021, an employee was on the back side of a truck, giving the driver directions to back up the truck to get close to two garbage cans. The truck struck him, causing fractures to his pelvic bone and sacrum. He was hospitalized.

An employee was crossing a street when a vehicle struck him, causing multiple fractures to his left leg/ankle. He was hospitalized.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was working in the food service warehouse when he experienced chest pain, difficulty breathing, and lost consciousness. The employee sustained carbon monoxide poisoning.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

An employee was working at her desk. She went to stand up and fell to the floor beside the desk. The employee sustained a hip displacement and required surgery.

An employee was processing wood boards at a chop saw when the saw malfunctioned and the blade cut her left hand and fingers. She was hospitalized and her little finger was surgically amputated.
