A crane was being tested in the ground support equipment area. An employee stumbled backward over the driver's side front outrigger, fell, and landed on his left wrist while trying to brace himself, resulting in a wrist fracture.

Two employees were in a boat pulling a 45-pound anchor out of the water by the rope. The rope tightened and as the employees were attempting to create slack in the rope, the injured employee began to slip. His hand was caught between the rope and the inside of the platform boat, resulting in amputation of the right middle finger with bone loss.

An employee was troubleshooting an aircraft from the cockpit. The employee exited the left side of the cockpit and stepped on the engine intake. When his weight shifted to his right foot, his foot slipped off the intake and he fell off the aircraft. He sustained a compound fracture to the wrist that required hospitalization and surgery.

An employee was riding a bike when a sign flew into his face causing him to lose control and fall off the bike hitting his head on the road.

While walking up a slight incline on a concrete surface, an employee lost balance and fell to the concrete, suffering head trauma and requiring hospitalization.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

On 7/31/2025, an employee was training to ride an ATV when it went into a draw and she went over the front of the ATV, landing on the ground. The employee sustained a fractured arm.

While stepping backward from a filing cabinet, an employee tripped on computers stacked in a walkway. The employee fell about 5 to 6 feet back, their head and neck striking a desk and chair. The employee suffered pain and swelling in the neck, a hematoma on the head, and a concussion.

A firearms instructor was performing routine training. Their pistol discharged while their right hand was in line with the muzzle and the employee's middle finger was partially amputated. The finger has been surgically reattached.

An employee was performing high dynamic movements while training. As he moved from kneeling to the next position, he suffered femoral neck fracture.

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee was preparing hot melted butter on a flat-top grill and placing it in small containers once it was melted. As she turned to set the container down, it slipped out of her hand and landed on the grill. Butter splashed up and hit her in the face. The employee suffered second-degree burns to her right eye and the right side of her head and face, as well as first-degree burns to her right forearm.

An employee pulled into the customer's driveway, parked, and exited the vehicle. He opened the sliding side door on the vehicle to retrieve a package. The customer began backing their vehicle out of their garage and it struck the employee in the back of his left leg. He was pinned him between the two vehicles, causing contusions and lacerations.

An employee reached into a hamper, picked up a package, and was hospitalized with a back injury that caused pain in his lower extremities.

On July 10, 2025, an employee tripped over a yellow pillar and fell to the floor. The employee was hospitalized with a fractured left femur.
