He reached for the handhold next to him and missed, falling 6 inches off the platform to the ground. The adjustable handle for the wing on the paving machine went between his left-hand fingers with the full weight of his body. The employee suffered a closed fracture of the fourth metacarpal bone in his hand.

While stepping backward from a filing cabinet, an employee tripped on computers stacked in a walkway. The employee fell about 5 to 6 feet back, their head and neck striking a desk and chair. The employee suffered pain and swelling in the neck, a hematoma on the head, and a concussion.

A firearms instructor was performing routine training. Their pistol discharged while their right hand was in line with the muzzle and the employee's middle finger was partially amputated. The finger has been surgically reattached.

An employee was performing high dynamic movements while training. As he moved from kneeling to the next position, he suffered femoral neck fracture.

On May 1, 2025, at approximately 11:00 AM, an employee was jump-starting the battery of a forklift and his back went out while he was bending down. The employee was hospitalized with a slipped disc and required surgery.
