An employee was taking concrete out from a cast mold when his middle finger became caught while trying to keep the "key-way" open, resulting in a left middle fingertip amputation.
An employee was preparing to remove concrete barriers from molds using a spreader bar that would be attached to a crane. The rigging attached to the spreader bar became caught on a small rebar hook that was welded to the center of the spreader bar.
