An employee was attempting to remove the drag link, also known as a steering arm, on a ready-mix concrete truck. He removed the front nut and was attempting to remove the rear nut but it would not come free. He then used a sledge hammer and struck the nut, which came off, causing the steering to swing and strike the employee on the left cheek and eye. The employee sustained a left orbital fracture, requiring hospitalization and surgery.

An employee was using a jack to place a small railcar back on track. The weight of the car shifted and the employee's right ring fingertip was caught between the jack and the railcar resulting in a partial amputation.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was exiting a loader and coming down the ladder. His hand slipped off the railing and he fell backward onto sandy ground, landing on his side. The employee was hospitalized with a fractured pelvis and a rotator cuff tear.

A concrete mixer truck driver was walking up the stairs of the slump rack. They fell from the slump rack platform and landed on the ground in the wash-out area. The employee sustained fractures to their left scapula and lower back vertebrae.

An employee backed their concrete truck into place to begin unloading it into a concrete pump unit. He exited the vehicle, and went to the rear to unload. When he began unloading, the truck rolled backward and pinned him against the pump unit. The employee sustained fractures to their pelvis, both femurs, both tibias and fibulas, and their left ankle.

A mechanic was changing out an air spring on a concrete mixer truck cab. The air spring was receiving higher air pressure than it was designed to use, because of a faulty level check valve. This increased pressure caused the air spring to burst. Its top portion separated and struck the employee's left forearm, causing a laceration and fracture. The employee was hospitalized, requiring surgery.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
