An employee was installing an actuator on a tilt skillet. While he was attempting to connect the motor with a pin, the motor dropped. His right finger was caught between the motor and the ground, amputating it from the nailbed to the fingertip.

A store employee was pushed by a shoplifter into the frame of the entrance door to the store. The employee's head struck the doorframe resulting in head injuries and contusions on her hand and arm. The employee was hospitalized.

A patient kicked a police officer in the leg, rupturing their patellar tendon. The police officer required hospitalization and surgery.

A cashier was hit in the face by a customer. The employee sustained injuries to his head and face.

An employee was making patient rounds at the hospital and stopped to talk with the charge nurse in front of the nurse's station. A patient reached over the nurse's station, grabbed a metal three-hole paper punch from over the desk, and proceeded to attack the employee with it. The employee sustained lacerations to the head and face, a fractured forearm, and a fractured little finger.

A teacher's assistant was attending to a student in the quiet room when they were pushed by another student. The employee fell to the floor and sustained a left hip fracture.

An employee was using a powered pallet jack. As he was maneuvering backward, his right foot slipped, causing the pallet jack to pin his right leg against a rack. He suffered a laceration between his ankle and his calf, as well as a fracture, and was hospitalized.

An employee was operating a high-density polyethylene (HDPE) fusion machine. The machine's carriage was directing a fitting that was being lifted from the machine when the employee's left ring finger was crushed in a pinch point, resulting in a fingertip amputation.

An employee was struck by a semi truck and pinned against a forklift while performing unloading activities. The employee was hospitalized with broken ribs and a torn left MCL.

An employee was on top of a railcar and was using a rod to push out some resin that had become lodged inside of the car. When he lifted the rod, it contacted a live, overhead wire and he was electrocuted. The employee sustained burns to 12-20 percent of their body.

An employee used a forklift to take a trash hopper to the dumpster. While driving back from the dumpster, the cord that had been used to open the hopper handle got caught under a wheel of the forklift causing the cord to tighten around the employee's left hand. The employee sustained amputation to the index, middle, and ring fingers.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
