An employee was cleaning debris from glue rollers when the employee's right thumb and index finger were caught between two rotating glue application rollers. As a result, the employee suffered a partial amputation to the right thumb and fracture to the right index finger.

An employee was attempting to clean rotating rollers with a rag when the employee's right hand and three fingers (index, middle, and ring) were crushed between the two turning rollers, resulting in the amputation of the index and middle fingers.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
