An employee was preparing to load a tanker truck. The employee slipped and fell from the top of the truck to the concrete floor and suffered a concussion, fractures and cuts/lacerations.
An employee was reinstalling an auger housing. The housing came down and injured the employee's right index finger, resulting in a fingertip amputation.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
