An employee was removing the back gate of a flat bed truck fitted with a removeable fence. The employee fell from the loading dock less than 4 feet to the ground. The employee later collapsed in the parking lot when trying to leave for the day and was hospitalized.

An employee exited his switcher truck to ask another driver where to drop his trailer when his truck started to move. He was trying to stop the truck when he was caught between it and another trailer. He suffered head trauma, broken ribs, and multiple lacerations, requiring hospitalization.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
