On November 21, 2019, an employee was mounting a center-ride double pallet jack. The pallet jack moved in reverse and the employee's leg was crushed between the pallet jack and a safety post. The employee's leg sustained internal damage that required surgery.
On 9/02/2020, at 12:10 PM, an employee was picking a case in an aisle behind a high-lift forklift. The forklift backed up and caught the employee against the rack. The employee was hospitalized with broken ribs.
